
Insurance Providers
We collaborate with three leading insurance providers: Aviva, AXA, and Vitality Health Insurance.
Aviva

1. Get a GP referral
You need a GP to refer you to a specialist, so they can give you a referral for physiotherapy treatment under Aviva health insurance. You can either use your NHS GP, Private GP, or Aviva’s Digital GP service.
They will decide if you need physiotherapy and refer you to seek physio care. Ask for an open referral so you can choose where you are treated. The other option is a named referral, where the GP refers you directly to us.
2. Contact Aviva to approve your claim
You can either call or log into your MyAviva app to start a claim. Ask about the details of which tests and treatments they will approve. Tell them that you wish to be treated by our
Aviva recognised Chartered Physiotherapist with details below:

3. Book an appointment

Share your membership number and Aviva physio claim details so we can let you know how many sessions will be covered by your insurance and how much you’ll need to pay.

4. Paying invoices
Aviva settles bills with us directly but only for approved sessions and conditions. You will be informed via the MyAviva app if you need to pay anything because of excess or benefit limits, and that will be paid to Aviva directly.

5. Ongoing treatment or more sessions
You must contact the help chat on MyAviva or call them to check if additional sessions can be covered by your plan.
Our physiotherapists can help you apply for more sessions if needed by sharing the codes for treatment or tests. We also create detailed medical reports that can be assessed by Aviva experts.
AXA

1. Find out your physiotherapy coverage details
Call AXA Health Insurance and confirm that your membership covers physiotherapy treatments. You’ll need your membership number to ask for such details. Confirm whether you need a GP referral for authorisation of physiotherapy treatment. Members who need a GP referral can get one within a day or two through AXA’s online/phone GP service or you can go through your NHS GP.

2. Inform them of your intent to get treatment from us
Please inform AXA that you wish to book an appointment with us and get authorisation.
You’ll provide them with the name of the therapist treating you the AXA registration number:

Keep your authorisation/claim number safe. Find out details of excess pay, and allowance (number of sessions covered by AXA or total amount AXA will pay for physiotherapy treatment).

3. Call us to book an appointment
You can book an appointment with us by contacting us through our contact form on this website or emailing contact@derehamphysiotherapy.com
Make sure you provide us with your AXA membership number, authorisation/claim number, excess details and allowance information.
You can provide multiple authorisation codes if you are claiming for more than one condition. We will book appointments and bill accordingly.

4. Pay for your session
Check how much excess you need to pay to our clinic. It depends on your policy type. You’ll get an email from AXA after authorisation detailing how much must be paid to us.
The excess amount varies depending on your membership/policy type but is typically around £100- £150 for most conditions. Since billing approvals take at least a few weeks, we’ll most likely send you a payment request for excess after your treatment has begun.

5. Ongoing treatment or more sessions
In case your treatment requires more sessions than what has been approved by AXA, you have two options: pay for the remaining sessions privately or request AXA for more sessions and request a new authorisation code.
We can help you request more sessions from AXA by providing clinical reasoning and documentation for more sessions.
vitality

1. Contact Vitality to approve your claim
Call Vitality to start a claim. They will ask you about your injury/concern. Please be advised that Vitality may compete an in-house triage call with one of their specialists to ensure that you are appropriate for Physiotherapy or may request that you contact your GP to receive a referral to Physiotherapy.
Tell them that you wish to be treated by our
Chartered Physiotherapist Luke Bartley who has Vitality provider recognition. Unlike Aviva & AXA, Vitality Healthcare do not issue provider recognition codes.

3. Paying invoices
Vitality usually settles bills with us directly but only for approved sessions and conditions. You should ask your provider if you need to pay an excess. Most policies through Vitality include an excess payment. An excess is a specified amount of money that Vitality expect you to pay for any outpatient service your receive before they will start covering you for further treatment that you receive over that amount.
Following payment of your excess to us in full, Vitality will cover 100% of your treatment. An excess is paid on an annual basis, from a start date nominated by Vitality.

Please note that any questions regarding the excess for your policy must be made to Vitality and we cannot contact them on your behalf to enquire for data protection/privacy reasons.

You must contact Vitality to check if additional sessions can be covered by your plan.
Our physiotherapists can help you apply for more sessions if needed by providing treatment reports.
